Community Survey - Public Health Plan

The City of Fremantle is preparing a Health and Wellbeing Plan to guide decisions that will improve health and wellbeing outcomes for residents.
We would love to hear from you about what you think the Plan should include, and ways to promote health and wellbeing in your community. The survey will take roughly 10 minutes to complete.

*Note: If you are a part of a local health provider/organisation, please fill out the Local Stakeholder Survey.

We are asking the public to: 

  • Learn more about the benefits and uses of a public health plan by reading the FAQs and Key Documents.
  • Submit your input by completing the Public Health Plan Survey.
  • Contribute feedback on what's currently working and provide best practice examples of public health strategy from other local governments or entities. 
  • Request to meet with city staff to discuss your thoughts on the public health plan via a 1:1 meeting
