Local Stakeholder Survey - Public Health Plan
The City of Fremantle is preparing a Health and Wellbeing Plan to guide decisions that will improve health and wellbeing outcomes for residents.
To reflect the thoughts, needs and aspirations of key local organisations we are asking you or a representative from your organisation to complete this online survey. We would love to hear what your organisation thinks the Plan should include, and ways to promote health and wellbeing in the Fremantle community.
This Plan will focus on the City's core business and functions, however if you would like to raise an issue where you feel advocacy and partnerships would benefit, please take advantage of this opportunity in Q14.
The survey contains 14 questions and should take around 10 minutes to complete. Please answer all questions honestly and to the best of your knowledge.
We appreciate your time is valuable and thank you for your assistance. Please feel free to forward this survey on to any other service providers who you feel would like to contribute to the health and wellbeing of the City of Fremantle residents.

- Learn more about the benefits and uses of a public health plan by reading the FAQs and Key Documents.
- Submit your input by completing the Public Health Plan Survey.
- Contribute feedback on what's currently working and provide best practice examples of public health strategy from other local governments or entities.
- Request to meet with city staff to discuss your thoughts on the public health plan via a 1:1 meeting.
